2021 AAVA AGM MEETING & ELECTION PRE-REGISTRATION FORM
AAVA members who will attend or participate in the 2021 Annual General Membership (AGM) Virtual Meeting and Election must pre-register as mandated by the Department of Human Settlements and Urban Development (DHSUD) on or before April 5, 2021. Prepare the following required documents: (1) copy of your valid ID with photo, preferably AAVA ID; (2) Copy of proxy form (if applicable); (3) copy of your proxy's valid ID with photo. The name and photo associated with your Google account will be recorded when you upload files and submit this form.

Property Address. Note: For multiple properties, please repeat this process for each and every property.
Upload a scanned copy of your valid ID with photo and Secretary's Certificate, if owned by a corporation.
